▶한글 성명 (Korean Name): to reflect the attendance register and academic information during the period of enrolment.

▶영문 성명 (English Full Name): include your degree, graduation certificate, etc.

1. 학적부 성명 기재 신청서(Application for Name Entry in Academic Records)

2. CONSENT TO COLLECT AND USE PERSONAL INFORMATION

3. Copy of your Passport
